If you want to pay by cheque (UK residents only) print the form and fill it out with as much detail as possible, then send it to the address below along with a recent photograph if you have one.

Horses, livestock £55;

Dogs, cats, small animals £48.

I will email you to confirm receipt of these items. Once assessed I will send the oils to you with detailed instructions on how to apply them, a report of the kinesiology test and other suggestions for holistic managment if applicable.
